We are pleased to announce that Kaiser Permanente is offering full scholarships to ALL symposia for community clinic physicians and staff. A full listing of all Kaiser Permanente Symposia is available at http://kpsymposia.com/. To register for this symposia and get a full scholarship, please follow the instructions below.

INSTRUCTIONS FOR KAISER PERMANENTE SYMPOSIA REGISTRATION

1. Go to the Kaiser Permanente Symposia website.

2. Register yourself (if registering for the first time).

3. Return to the main page of the website.

4. Indicate the symposia for which you wish to register by clicking "Register" (on the far right).

5. Answer all necessary fields on the registration page. When you reach the question "Do you work for Kaiser Permanente or are you a Community Colleague?", indicate "Community Clinic/Public Hospitals (code required)".

6. In the "PAYMENT" section, fill in "Promotional Code" as CBSYMPOSIA.

7. Due to an idiosyncrasy in the nature of the system, in the "PAYMENT" section it will also be necessary for you to indicate "Payment Method" as "Credit Card". Simply fill in the number "1" (or any number other than "0") sixteen (16) times, and fill in credit card expiration date. [Note: Personal credit card information is not needed.]

8. The total charge will be $0.00 and you will be registered for the event with full scholarship.
