2009-2011 Objectives
• 90% of 137 primary care clinics are using disease registries for patients with diabetes both at the point of care and for regular follow-up care.
• Improve diabetes outcomes through use of disease registries whereby the most recent HbA1c is less than 8% for 90% of diabetic patients and 80% of diabetes patients have well-controlled blood pressure.
• Public hospitals scoring in the top 10% nationwide on 30 publicly reported CMS core quality measures.
• Palliative care services established in 11 public hospitals, providing humane and efficient end-of-life care.
