QLA Webinar Series

The QLA Webinar Series provides an opportunity for CAPH-members to learn more about QLA honorees and  their various programs and approaches. Featuring three QLA programs a year, these webinars give participants an in-depth look into how the initiatives were implemented, and enables connection and discussion among colleagues for how to make improvements within their own institutions.

On April 7, San Mateo Medical Center presented their Community Health Network for the Underserved. As a result of their work, San Mateo County has seen expanded involvement in services to the indigent and Medi-Cal populations from 10 major private provider organizations serving the County, $12 million in estimated value of partnerships and/or new funding to support the Network, and improved geographic access for hundreds of low-income women delivering babies within their county. Kicking off the web-conference series on March 26, 2010 was Kern Medical Center’s Impact of Clinical Pharmacists Involvement in Direct Patient Care as Mid-Level Practitioners. Demonstrating reduced hospital admissions and ER visits by 78.9% for diabetes-related complications and a reduction in drug-to-drug interactions by 40%, KMC's Ryan Gates, Pharm.D. shared the essential details of their successful initiative.  Click here to playback the presentation.

The second webinar on April 21st highlighted San Francisco General Hospital/UCSF’s Automated Telephone Self-Management Support Model for Diabetes program, which earned their organization a Top Honors award.  The presentation was conducted by Dean Schilllinger, MD, UCSF Associate Professor of Clinical Medicine Director and UCSF Center for Vulnerable Populations Chair and his associate Margaret Handley, adjunct professor at UCSF and co-director of the UCSF collaborative research network.  Click here to playback this presentation, or view the PDF here.

The third and final webinar was held on May 26th and featured UC Davis' Toolkit for Constructing the Medical Home and Improving the Patient Experience.  Angela Gandolfo, Practice Manager for the Family Practice Clinic presented the model they developed for creating a medical home.  Click here to playback the presentation.
