Helping public hospital systems improve patient and community health

California Health Care Safety Net Institute (SNI) is dedicated to fostering quality improvements and system innovations in California’s public hospitals and health care systems. To do this, SNI connects public hospital systems with research, training, education and best practices to help them improve care for those who need it most.

Priority Areas

	Improving Chronic Care The Initiative seeks to increase the number of chronic care champions in public hospital systems and to assist these champions in transforming their individual efforts into systematic change.
	Addressing Health Disparities SNI is supporting the development of public hospital systems into centers of excellence for quality care with the aim of reducing and eventually eliminating healthcare disparities.
	Enhancing System Capabilities New technologies and other innovations offer public hospitals and health systems powerful new opportunities to increase efficiency and foster community health.
SNI is an education and research affiliate of the California Association of Public Hospitals and Health Systems (CAPH)
