SNI, the Primary Care Development Corporation and Coleman & Associates launched
a Patient Visit Redesign Collaborative with funding from the California
HealthCare Foundation. This highly structured program aims to decrease the time it
takes to complete a primary care visit to under an hour, and to increase provider productivity—without
reducing the time patients spend with their doctor. Nine primary care teams from five
CAPH member public hospital systems throughout California participated in this program. They
achieved:
- A 45% decrease in patient visit cycle time
- A 36% increase in provider productivity
Now SNI and Coleman Associates are helping the teams spread their successful
redesigned patient visit models to other clinics throughout their systems. SNI and Coleman
Associates are also launching a new redesign project for a second group of public hospital
clinic teams.
Currently, SNI is sponsoring two Collaboratives for Patient Visit Redesign:
The Spread Collaborative:
Begun in Spring 2006, SNI and Coleman Associates
are helping the teams from the 2005 Collaborative spread their successful, redesigned patient
visit models to other clinics throughout their systems.
New Patient Visit Redesign Collaborative:
The new Patient Visit Redesign Collaborative will be starting
in September.
The participating systems are: Alameda County (2 clinics), High Desert
(1), Olive View (2), King/Drew (1), Contra Costa (2), Kern (2), and
Riverside (1). These 11 clinics bring the total number of CAPH
clinics to participate in the SNI/Coleman Associates Redesign Initiative
